Partial telehealth - Locums Home Health APP Need in North Carolina

Burlington and Greensboro, NC region

Work Details:
- Patients per day: Combo of telehealth and home visits: 6pt/day for home visits (Apts approx. 45 min); 8pts per day for telehealth (All equipment provided)
- Conducts physical examinations, diagnosing and treating illnesses, managing chronic conditions, engaging in advanced care planning discussions, and coordinating care with other healthcare providers.
- Working out of (Greensboro and Burlington): Traveling to make home visits in the surrounding 5 counties.
- They will be working in a regionalized area per day but the areas can change from day to day.

Requirements:
- License required: NC license required
- MUST HAVE: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
- Minimum experience required: 2-3 years experience in an acute care, home health, long-term care, or outpatient/office setting required. Palliative care experience preferred.
- Credentialing Timeframe: 1-2 weeks

• M-F 8a-5p (40hr/ week)
