Job Description:
Community Health Center in Troy New York Seeking a Physician
• Willing to consider a FP or IM
• Provider would ideally be seeing a mix of children and adults, but mostly adults
• Must be Board Certified
• 1+ year experience needed
• Mon-Fri
• 8a-5p
• PT possible, but must be willing to do 3 days a week
• Basic primary care: sick and well visits, vaccines, chronic care management, etc.
• Will be solo provider onsite most days
• Full support staff: MA's and LPN's
• EMR: Athena
• Must have active license, DEA, and Medicaid
• Credentialing timeline: 30 days or less
• T&L: Possible for the right candidate
• Ideal candidate would have community health experience

• Mostly adult patients- basic primary care
• Must at least do 3 days a week
• Community health experience is a plus!
