✅ Qualifications
✔ MD or DO from an accredited medical school
✔ Active New York State Medical License
✔ Board Certified or Board Eligible in Internal Medicine
✔ Experience preferred in geriatrics, nursing homes, or long-term care
✔ Proficient with EHR, chronic disease management, preventive care
✔ Compassionate, culturally competent, and strong communicator

✅ Key Responsibilities
✔ Diagnose, treat, and manage acute and chronic conditions
✔ Prescribe medications and therapies to optimize patient health
✔ Monitor patient progress and adjust care plans accordingly
✔ Educate patients and families on wellness and disease prevention
✔ Administer immunizations
✔ Maintain thorough and accurate medical records
✔ Refer patients to specialists as needed
✔ Support patients and families in long-term care settings
✔ Participate in clinical research and innovations
